Several doctors have called it “inconceivable” that former President Joe Biden’s aggressive prostate cancer went unnoticed by his medical team — even as Donald Trump Jr., Biden’s eldest son, questioned whether the diagnosis was part of a “cover-up.”
The 82-year-old former president received the diagnosis on Friday after doctors discovered a “small nodule” on his prostate during a medical exam earlier this month. His spokesman revealed Sunday that the cancer had spread to his bones.
Biden’s medical team reported that the cancer was graded with a Gleason score of 9, indicating one of the most aggressive forms of the disease.
Several medical experts quickly questioned how the former president’s cancer was diagnosed at such a late stage—particularly since routine blood tests, recommended for men over 50, can typically detect prostate cancer early.
Dr. Howard Forman commented on X, saying, “It is inconceivable that this was not being monitored before he left the Presidency.”
“Gleason grade 9 would have caused elevated PSA levels well before this diagnosis, and he must have had multiple PSA tests previously. This is unusual,” Dr. Forman added. “I wish him well and hope he can maximize his quality of life.”
Dr. Steven Quay expressed similar concerns, noting that Biden’s type of cancer is typically one of the easiest to detect in its early stages.
“The PSA blood test indicates the rate at which cancer cells are growing. Even with the most aggressive form, it typically takes 5 to 7 years without treatment before the cancer becomes metastatic,” he explained.
“Therefore, it would be considered malpractice for a patient to first be diagnosed with metastatic disease in May 2025,” he added.
“It’s highly likely he had prostate cancer throughout his time in the White House, yet the American public was kept in the dark.”

Medical analyst Dr. Marc Siegel also weighed in during an interview with Fox News, expressing surprise at the aggressiveness of Biden’s illness given his high-profile status.

“What I want to know is how did Dr. Jill Biden miss stage five metastatic cancer—or is this yet another cover-up???” he wrote on Truth Social.
Though referred to as “Dr. Jill Biden,” the former first lady holds a doctorate in education from the University of Delaware and is not a medical doctor.
Biden’s team revealed that the diagnosis came after the former commander-in-chief began experiencing urinary symptoms.
“Although this is a more aggressive form of the disease, the cancer appears to be hormone-sensitive, which allows for effective treatment,” his office stated.
“The President and his family are currently consulting with his doctors to review treatment options.”
